Successful Treatment of Herpes Simplex Virus (HSV)-1-associated Hemophagocytic Lymphohistiocytosis (HLH) with Acyclovir: A Case Report and Literature Review

چکیده
Hemophagocytic lymphohistiocytosis (HLH) associated with herpes simplex virus (HSV)-1 infection (HSV-1-HLH) is uncommon and is potentially fatal without appropriate treatment. We herein report the case of an adult patient with HSV-1-HLH who was successfully treated with acyclovir. A 69-year-old man developed fever, pancytopenia and liver enzyme elevation after the resolution of pneumonia. These findings and the presence of hemophagocytosis in the patient's bone marrow were consistent with a diagnosis of HLH. The patient was diagnosed with HSV-1-HLH based on the results of a polymerase chain reaction (PCR) for HSV-1. The early administration of acyclovir improved his clinical symptoms and laboratory results within two weeks. In the present case, the rapid and precise diagnosis facilitated the successful treatment of HSV-1-HLH.

Acquired Hemophagocytic Lymphohistiocytosis: a Case Report of Two Neonates
We report two cases of Herpes Simplex Virus (HSV) induced hemophagocytic lymphohistiocytosis (HLH). These cases highlight that neonates with HSV may have concurrent secondary HLH, a life threatening condition in which prompt diagnosis and treatment can drastically improve survival. Therefore, a low index of suspicion is needed to send, cost effective, sensitive screening blood tests for HLH in ...
متن کاملSuccessful Treatment of Corticosteroid with Antiviral Therapy for a Neonatal Liver Failure with Disseminated Herpes Simplex Virus Infection
Background Herpes simplex virus (HSV) infection carries one of the poorest outcomes of neonatal liver failure (NLF). Neonates with disseminated HSV infection can develop hemophagocytic lymphohistiocytosis (HLH), and occasionally need orthotopic liver transplantation. Early interventions may be critical for the cure of NLF. Case Report We describe herewith a 6-day-old neonate with fulminant hepa...

Fingolimod is an oral, disease-modifying therapy used to treat multiple sclerosis (MS). However, severe viral infections, including disseminated varicella-zoster virus infection, herpes simplex encephalitis, varicella-zoster virus encephalitis and vasculopathy, or progressive multifocal leukoencephalopathy, have been reported during fingolimod therapy. Hemophagocytic lymphohistiocytosis (HLH), ...
متن کاملLETTER FROM THE FRONTLINE First Description of Single-Pass Albumin Dialysis Combined With Cytokine Adsorption in Fulminant Liver Failure and Hemophagocytic Syndrome Resulting From Generalized Herpes Simplex Virus 1 Infection
Acute liver failure (ALF) is a rare, life-threatening complication of herpes simplex virus (HSV) infection that can occur in immunocompetent patients. Liver transplantation (LT) is the ultima ratio in cases of ALF that progress despite antiviral treatment. To bridge the time until LT, extracorporeal liver support with the Molecular Adsorbent Recirculating System (MARS; Gambro, Lund, Sweden) has...

Case presentation • Liver biopsy revealed acute liver cell necrosis due to herpes simplex virus type 1 (HSV-1) • Despite antiviral therapy liver failure progressed and patient was transferred to ICU • Rapid development of MOF with hepatic coma, severe coagulopathy, acute anuric renal failure, respiratory insufficiency and arterial hypotension • Patient was listed for highly urgent liver transpl...
